PLEASE READ BEFORE SENDING YOUR FORMS!!!!!!!!
Please do not drop off any registrations to the Central School Office or send with your child to school. We will also not accept any registrations without payment.
REGISTRATION IS NOT GUARANTEED BY SENDING VIA EMAIL OR MAIL. SPACE DOES FILL UP QUICKLY AND IS FIRST COME, FIRST SERVE. WE WILL CONTACT YOU TO CONFIRM BY EMAIL THAT WE HAVE RECEIVED YOUR FORMS AND THAT YOUR CHILD IS ALL SET WITH THE DAY(S) CHOSEN. IF WE ARE FULL WHEN WE RECEIVE YOUR SIGN UP YOU WILL HAVE THE OPTION TO BE PUT ON A WAITING LIST. WE WILL CONTACT YOU IF THIS HAPPENS AS WELL.

All registration forms must be filled out completely and turned in with full payment BEFORE your child starts their first day with us.
CLUBHOUSE KIDS WILL NOT OPEN ON THE FIRST DAY OF SCHOOL. START DATE: MONDAY, SEPTEMBER 9TH
✔ A registration fee of $40.00 per child and advance payment (deposit) is required. No registration will be accepted without payment. A deposit is required for all who register, even for drop-ins. Please see NEW RATES on forms.
✔ All previous account balances must be paid in full BEFORE registering your child in this program.
✔ PARENT HANDBOOK: Please read carefully and become familiar with our policies before signing up your child. Please sign agreement noting you have read and understand our policies.

★ WE NEED YOUR CHILD'S IMMUNIZATION RECORDS WITH YOUR REGISTRATION , AS WE ARE REQUIRED TO HAVE THEM ON FILE. ★ WE NEED THIS BEFORE YOUR CHILD STARTS THEIR FIRST DAY, NO EXCEPTIONS. YOUR CHILD CANNOT START IF WE DO NOT HAVE THEM.
